The referral form below places the individual on a list for review and our team will follow up. Due to limited capacity, submitting a referral does not guarantee acceptance into the DASH program.
If the individual you are referring needs immediate assistance, please note that DASH Network is not an emergency shelter. For urgent needs, we encourage you to explore additional local resources, some of those are listed here.
DASH operates by appointment only and is unable to accommodate walk-in visits.
